Volunteer Statement

The Salvation Army aims to provide a safe and secure environment for all volunteers, staff, and men, women and children who participate in Salvation Army programs and use our facilities.

By filling out the Volunteer Registration Form, I do hereby represent to The Salvation Army that all subsequent statements are true:

• In my prior experience, volunteer or otherwise, I have never used a name other than those listed.
• I am physically able to perform and complete the jobs I have volunteered for without limitations or special conditions.
• I have never been accused or convicted of child abuse or a crime involving actual or attempted sexual molestation of a child or adult.
• I consent to a background check and release all listed organizations and individuals from any liability that may result from their furnishing information to The Salvation Army. I likewise waive any right to inspect any records containing such information.
• I am aware that, as a volunteer, I represent The Salvation Army. I understand that The Salvation Army is a highly-respected and professional organization based on the Christian principals of compassion, charity and non-discrimination, and I will conduct myself in a manner consistent with these policies and principles.
• I assert that all information or statements made here are true. I understand that if this information is found to be untrue, my assertion may be punishable under the laws relating to perjury.
• Offering Consent to Publication, I give my permission to The Salvation Army to print, reproduce, and/or republish any portraits, pictures, photographic prints or other representations of me or images in which I may appear. I understand that I may rescind this Consent to Publication at any time.
